Personalised Care and Support Plan update event

Since January this year the Universal Care Plan (UCP) has become an extended Personalised Care and Support Plan. It is a digital plan that is completed online by care and health staff,  and can be viewed by 111 service, ambulance staff,  hospitals as well as Local Authority Social Workers.

UCP’s can significantly change health and care outcomes for people with a learning disability across London. As Learning Disability support staff, you often play the biggest role in supporting health and maintaining wellbeing.  Many staff now complete the hospital passport but rightly recognise the limitations and frustrations that can be associated with it.  A digital plan is written once, updated in real time and then seen by health and care staff across London.

The webinar will cover all you need to know about UCPs including the difference between UCP and Hospital Passport. Our colleagues from our pilot sites will share practice examples. We will share resources,  and there will be time for Q & A.
